View COVID-19 Entrance and Visiting Restrictions


At this time, your family and friends are discouraged from visiting to reduce the spread of COVID-19. However, when needed, only one adult visitor at a time is allowed in the hospital. General visiting hours are 7 a.m. to 8 p.m. Special visiting hours are established for the Intensive Care Unit (ICU). Please call 815-876-2287 for more information. Quiet times (no visitors) will be shared when you call.


The following is a list of main entrances and their hours of open access.  Outside of these hours, access is limited to the Emergency Department/Outpatient Registration Entrance.

5th Street Entrance (main entrance)

Medical Office Building Entrance (north building / specialty physician clinics)
  Monday through Friday:  6 a.m. to 5 p.m.

Emergency Department / Outpatient Registration Entrance (west entrance)
  Open 24/7.

You will find our cafeteria on the lower level of the hospital. It is listed in the elevator as (LL).  The cafeteria offers breakfast from 7:30 a.m. to 9:30 a.m. and lunch from 11:00 a.m. to 1:00 p.m.  The Park Avenue Café offers a “Made to Order” menu on Saturdays, Sundays and Holidays from 11:00 a.m. to 1:00 p.m.  

Self-checkout items are available for dinner and during all non-serving hours (no cash payment).  Self-checkout items are packaged in as grab-n-go with barcodes.


Vending machines are available in the cafeteria, in the outpatient/emergency area on the first floor and the second floor waiting room. A pop machine is located on the fourth floor of the Physician’s Building (Medical Office Building) by Human Resources. Candy and snacks are also sold in the Gift Shop.

Call in advance if possible, to assist with getting you home to Princeton or a surrounding community. BPART (Bureau & Putnam area rural transit) provides pickup and drop-off (handicap ramp or lift on every vehicle), Monday through Friday at Perry Memorial Hospital’s outpatient entrance and the medical office building. They are provided at the top of every hour from 9 a.m. to noon and again at 2 p.m. and 3:30 p.m. Call to schedule a ride:  877-874-8813,


If you need assistance unloading or loading a loved one, the Perry team is ready to assist. If you are located in the Medical Office Building entrance on Park Avenue East, contact Medical Rehabilitation for assistance. If you are at the outpatient or emergency entrance, contact the Emergency Department for assistance. Once an assessment is made, the operator may be requested to call the Lift Assist Team, available 24 hours a day.
