Visitor Information


Your family and friends are encouraged to visit. However, only two visitors at a time are allowed in your room. General visiting hours are 7 a.m. to 8 p.m. Special visiting hours are established for the Intensive Care Unit (ICU) please call 815-876-2287; quiet times (no visitors) will be shared when you call.


Perry is building an integrated healthcare delivery system for our communities’ future health and wellness.  The goal:  improve the quality of life for those living in our community. Perry Memorial Hospital is committed to the safety and security of our, patients, visitors and employees.  Everyone who enters our facility can do so secure in the knowledge patient care and well-being is Perry’s primary concern.

Safety is paramount and the Perry team routinely identifies opportunities to minimize safety risks.  Perry is instituting a few new best practices while you visit or receive care at Perry.  Following is a list of main entrances and their hours of open access.  Outside of these hours access is limited to the Emergency Department/Outpatient Entrance and East Hospital Visitor Entrance.
The North Physician Clinic/Medical Office Building, Park Avenue East entrance
Monday through Friday:  6 a.m. to 8 p.m. (weekend hours as needed by scheduled events)

The East Hospital Visitor Entrance, 5th Street entrance
Monday through Sunday: 6 a.m. to 9 p.m.

The West Emergency Department/Outpatient entrance
Open 24/7. After 9 p.m. All persons  presenting to the hospital will be asked to identify themselves prior to admittance by the use of the red phone at the Main Emergency Department/Outpatient entrance.

You will find our cafeteria on the lower level of the hospital. It is listed in the elevator as (LL). The cafeteria offers breakfast from 7 a.m. to 10:15 a.m., lunch from 11 a.m. to 1:30 p.m., and dinner from 4 p.m. to 6:15 p.m. The Park Avenue Café offers their “Made to Order” menu on Saturdays, Sundays and Holidays.


Vending machines are available in the cafeteria, in the outpatient/emergency area on the first floor and the second floor waiting room. A pop machine is located on the fourth floor of the Physician’s Building (Medical Office Building) by Human Resources. Candy and snacks are also sold in the Gift Shop.

An ATM is located on the first floor of the Medical Office Building.


Call in advance if possible, to assist with getting you home to Princeton or a surrounding community. BPart (Bureau & Putnam area rural transit) provides pickup and drop-off (handicap ramp or lift on every vehicle), Monday through Friday at Perry Memorial Hospital’s outpatient entrance and the medical office building. They are provided at the top of every hour from 9 a.m. to noon and again at 2 p.m. and 3:30 p.m. Call to schedule a ride:  877-874-8813,


If you need assistance unloading or loading a loved one, the Perry team is ready to assist. If you are located in the Medical Office Building entrance on Park Avenue East, contact Medical Rehabilitation for assistance. If you are at the outpatient or emergency entrance, contact the Emergency Department for assistance. Once an assessment is made, the operator may be requested to call the lift assist team, available 24 hours a day.
