The My PerryHealth portal offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the internet to view information such as details about your visits to Perry Memorial Hospital, medical history, allergies, lab results and more.
Patients have two options for signing up for My PerryHealth Portal.
1) Patients can enroll during a visit to Perry Memorial Hospital by talking with Registration Staff. You will need proof of identification and a valid email address.
2) Patients can self-enroll using the button above.
If you would like to sign up for My PerryHealth portal and are NOT presenting for services, please choose the My PerryHealth Self Enrollment button.
You will need to enter your information and create a user name and password. You will also need your MRN (medical record number). To obtain your MRN, you will need to contact the Health Information Management department at (815) 876-4414, Monday through Friday, 6:30 a.m. to 4:30 pm. and provide the last four digits of your social security number and your date of birth.
Once you have contacted Perry Memorial Hospital, you will receive an email that will invite you to the My PerryHealth Patient Portal with a link to set up your account.
After registering in person at Perry Memorial Hospital, you will receive an email from email@example.com with instructions on how to create your user ID and password.
For current needs, click on the “My PerryHealth” button. For access to past information dated through May 20, 2018, please click on “Previous Portal.” In the future, all older information will be moved to new “My PerryHealth” button.
No, the My PerryHealth Portal is a free service to Perry Memorial Hospital patients. You will not be charged to enroll.
Opening the link in the Patient Portal New User email, you will complete the New User Registration. You will provide your First Name, Last Name and Date of Birth. It is very important that the spelling of your names match the hospital records. In addition, you will create your secure personal User ID and Password. Instructions on how to complete My PerryHealth creation.
As long as you have internet access, you will be able to access the MyPerryhealth Portal located on the Perry Memorial Hospital website or by visiting https://perrymemorial.iqhealth.com.
After successful login to your My PerryHealth Patient Portal account, you will be able to navigate a dashboard on the left side of the page for various components of your record.
Perry Memorial Hospital takes great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal IDs and passwords. Following the registration of your User ID and Password, you will choose and answer three security questions. These security questions will be used if you forget your User ID or Password.
The My PerryHealth Patient Portal is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given to your health records and will never be sold or leased.
The My PerryHealth Patient Portal allows Perry Memorial Hospital patients access to a summary of both inpatient and outpatient visits. Although it is not a complete view of the patient’s entire medical record, the summary contains the follow types of information: Care Team, Vital Signs (Inpatients only), Allergies, Procedures, Lab Results, Medications Administered and Discharge Information.
