Working together, your Care Team, comprised of physicians, nurse practitioners, nurses and supporting staff, coordinate with you to ensure yor Perry experience is a positive one. From the simplest to the most complex conditions, the care you receive will be in the right place, at the right time, and in the manner that best suits your needs and goals.

Perry's Care Teams are designed with you at the center. They know the importance of collaborating with each other and with your specialist providers to help you navigate treatment of any chronic conditions and close the gaps on your unique healthcare needs. Your Care Team will assess your health, review your goals, and help you achieve the best possible outcomes.
Throughout your care, your Care Team will provide guidance on the importance of preventative care, including diet, exercise, immunizations, and health screenings. For all ages, your Care Team will manage your healthcare journey--including health history, test results, and visit information--using our electronic medical records system.

Yes! The Perry Memorial Family Health Clinic includes Telepsych services for people of all ages, including diagnosis, short-term counseling, and referrals for long-term care. For Medicare-eligible patients, your Care Team may also refer you to Perry's Senior Behavioral Wellness program.
The Care Teams in the Perry Memorial Family Health Clinic see patients with scheduled appointments Monday through Friday.
Monday through Friday: 7 a.m. to 5:30 p.m.
Lunch Hour: Noon to 1 p.m.
Evenings by appointment only
If you are in urgent need of care on a weekend or after clinic hours, visit Perry Memorial Prompt Care, or, for more severe health concerns, the Emergency Department.
Yes! HealthE-Visits are available for anyone who prefers to stay connected through a virtual phone or video appointment. Depending on your needs and health conditions, your provider may recommend an in-person appointment instead. Learn more about HealthE-Visits.
The Family Health Clinic is located in a brand new, easy-access facility across the street from the Medical Office Building. The clinic includes 15,200 square feet of space, with 21 exam rooms and 2 procedure rooms. It also includes one bariatric and vitals room with a wheelchair scale.
